2008 Golf Outings

Support Camp MACK Register for the 2008 Golf Outings Sat., Aug. 9th at McCormick Creek Golf Course in Nappanee (Address/Travel directions with confirmation) You can count on my/our support in the following ways: SPONSORSHIP: (Please indicate wording for sign on reverse side.) (Sponsor in time for our May outing, and your sign will be also be displayed at the August outing - both outings for one price!) _____ PAR level -- $100 contribution (listing in program) _____ BIRDIE level -- $200 contribution (tee box sponsor sign) _____ EAGLE level -- $400 contribution (tee box sponsor sign & 2 players at one outing) _____ HOLE-IN-ONE level -- $1000 contribution (Tournament Sponsor sign & 4 players at one outing) GOLFERS $65 per person per outing includes golf, cart, prizes, food and beverages. ____ August 9 at McCormick Creek Golf Course at Nappanee.
